All criminal justice internship documents are available in a WORD file labeled "Internship Documents.doc" located at the bottom of this page.

Page 1 (Cover Page)
The first page is a cover page with most of the pertinent information concerning your internship. After completing this page, print three copies of the page. Keep one copy for your reference, give one copy to your faculty coordinator and give one copy to your site supervisor.

Save file as a new document
The fields on the first page are linked to other fields throughout the document. By saving the document once the cover page is completed, you save the fields populated throughout the document.

Page 2 (Internship Agreement)
The only fields to be completed on this page are in the following statement:

"[NAME] will work approximately ___ hours per week during the period beginning on or about ____ and ending on or about ____. [NAME] will receive [#] credit hours for successful completion of this work experience"

Fill in the dates your internship begins and ends, along with the approximate number of hours you will be expected to work each week. Print one copy of page 2, acquire all of the necessary signatures and return thecompleted form to your Faculty Coordinator.

Page 3 (Learning Objectives)
You will need to discuss your learning objectives with your site supervisor before completing this form. Once you begin typing your objectives, you will see that these fields will expand to accommodate whatever text you wish to include. After completing the form, this page also needs to be signed by the appropriate persons and returned to your Faculty Coordinator.

Page 4 (Daily Journal) and Page 5 (Weekly Summary)
These are self-explanatory. The Daily Journal is to be completed each day of your internship and a Weekly Summary is to be completed each week. These text fields will also expand as you type. Remember that these forms must be turned in weekly.

The Weekly Summary needs to be signed by your Site Supervisor.

You do not necessarily need to save the document again. You will use pages 4 and 5 each week.
